This SMS checkbox is separate from general terms or privacy acceptance, unchecked by default, and applies only to the person submitting the form as the primary contact. A submitter cannot opt in another or secondary contact.

This public reviewer document reproduces the exact SMS consent block used on an offline intake, retainer, or communication authorization. The recipient—not firm staff—completes the fields, checks the separate box, signs, and dates the document.

Keyword re-opt-in is not an initial opt-in method. After STOP, the same sender thread displays “Reply START to opt back in” together with its number. Only that prior recipient may reply START or YES. STOP, UNSUBSCRIBE, or CANCEL opts out; HELP or INFO returns help instructions.

Mobile information and SMS consent

Mobile phone numbers, SMS opt-in data, and SMS consent are not shared, sold, rented, or disclosed to third parties or affiliates for marketing or promotional purposes. We share this information only with service providers that operate the Ready Text SMS program (such as messaging carriers and platform vendors), solely to deliver, support, and secure the requested messages; those providers may not use it for their own marketing or promotional purposes.
